ATTENTION ALL TEAMS
CHECK-IN INFORMATION:
Team members do not need to attend Team Check-in; one adult with all of the required team documentation is required to attend.

Check-in will be at the Specht Road Fields. Teams with 9 a.m. games on Saturday morning are asked to arrive for check-in at 7:30 a.m.

TEAM REQUIREMENTS
All teams are required to bring the following items to Team Check-in:
ROSTER: A current Fall 2008 Official Roster signed by a responsible officer of the Member Association listing all players rostered to the team as of November 1.
PLAYER ID CARDS: Current Fall 2008 Player ID Cards produced from the Affinity registration system. Each player must have a properly prepared ID Card containing the required information (name, date of birth, ID Number, Team Code and Team Name); be signed by the player and registrar; be laminated; and contain a recent photo of the individual named on the card. PLEASE place the ID Cards in alphabetical order for a faster check-in process.
ADULT PARTICIPATION PASS: All adults must have a current 2008/2009 (YELLOW) Adult Participation Pass confirming they are properly registered, have cleared the required background check, and hold a coaching license or are within their first year of coaching. Any adult with a GREEN Pass will NOT be allowed on the sideline as that pass was valid only for the 2007-2008 seasonal year. Four adults per team will be issued SIDELINE PASSES that will allow them to be on the team sideline during games. ALL adults on the sideline must have their SIDELINE PASS visible at all times.
PENALTY POINT REPORT: A Penalty Point Report is required for each team. For teams who participated in a District Tournament, an updated copy of your report has been forwarded to the appropriate State Tournament Coordinator. The report must include the type of penalty points (yellow or red cards) accumulated by each player, coach, assistant coach or trainer on the team as well as the date that any required suspension was served and must be signed by the designated league adminitrator.
